Specialty Care Services:

Our Specialty Care Services is what sets up apart from others within the community.  Our staff and caregivers are highly trained and qualified in caring for individuals living with ALS (Lou Gehrig Disease), Multiple Sclerosis, Parkinson and Spinal Cord Injuries. Veteran Care Services: 

Our Veteran Care Services allows our veterans to enjoy a quality of life while maintaining as much independence within their home through a personalized care plan for their daily needs. 

​

Overnight Care:

Overnight care is an option with a referred caregiver from Palmetto Family Homecare. This services involves a caregiver assisting with a few hours of care before bed and after a good night’s rest. Caregivers usually request that they receive at least 8 hours of sleep, but they are there to assist you during the night if your loved one is unable to sleep or need assistance at night. Having a caregiver in the home as your loved one sleeps gives peace of mind and security while you rest, enabling you to wake up fresh to go about your day.

 

Facility Sitter Care Services:

Our Facility Sitter Service can be provided when your loved one is in the hospital, nursing home and/or a rehabilitation center.  This service provides a peace of mind and a sense of reassurance to the client and family. The caregiver can provide companionship, assist the facility staff in monitoring a client's health status, preventing falls and encouraging nutritional intake.

Rehabilitative & Post Hospitalization Care:

Our rehabilitative services can help restore, rejuvenate and provide balance while you recover or rehabilitate at home from an illness, injury, or surgery.  This service can help you regain confidence and independence faster by allowing you to focus your time and energy on recovering instead of the daily to-do lists. We can assist you with your personal care, homemaker & home management needs, errand running, or general companionship.

Transition Care Services:

Our transition care program helps with discharge coordination and execution; whether you are leaving from the hospital, rehabilitation facility or nursing home.  Having a smooth discharge process is key to recovery and reduction of facility readmission. Prior to discharge, one of our Care Coordinators will assist in developing your homecare needs along with coordinating with facility social workers and/or discharge planners to make sure all the needs of your loved one are met.  We will also assist with medication management, Physician follow up visit coordination, nutrition management, supervision and documentation of care.

End of Life Care Support: 

Our end of life support care provides an additional support team to the family when dealing with a loved one who is nearing and/or at the end stages of life.  It is very important to us for our families to enjoy the remaining time they have with their loved one; and with our team in place it will allow you to focus on the more important things and create lasting memories.  We work closely with other providers and hospice care providers within the community to ensure that the entire family is comforted with quality and compassionate care.

Medication Management:

Our Medication management service is monitored and performed by an RN. This service reviews all prescription medication on a weekly basis, maintains and updates list of all current medication, set up weekly medication tray, assist with medication refills and appointments and communicating with physicians for any concerns or updates.

Care Management:

Our goal is to ensure that all of our clients and their family is satisfied and well taken care of.  The Care Management team of Palmetto Family Homecare will develop a care plan of service and hours needed based on your specific need and request.  We will also do weekly and monthly check-ins via phone and in person.
